The carrier profile report is a compilation of the carrier profile that captures a motor carrier's previous 12-month record of convictions, accidents, and Commercial Vehicle Safety Alliance (CVSA) inspections. It is made up of 5 distinct sections that are as follows:
Section 1: The cover page
The cover page (pdf, 55 kb) states that the report is a Saskatchewan carrier profile. It provides the following:
- name of the company or an individual name
- the National Safety Code (NSC) number assigned to the carrier
- the Customer Numbers reported on the vehicle registrations
- the date of the profile report
- the safety rating
Section 2: The carrier summary page
The summary page (pdf, 101 kb) provides a summary of information regarding the company such as:
- name and address
- the NSC number
- the average fleet count of registered NSC vehicles over the last 24 months
- an audit date and audit results
- NSC safety rating
It also contains summarized safety infraction information on each of the 3 safety measures for convictions, at-fault accidents, and CVSA inspections that affect the safety threshold levels reported on this page.
The summary page is available to the general public upon request to CarrierSafetyPrograms@sgi.sk.ca.

Detailed accident information affecting the safety levels reported on the carrier profile.
Section 4: Convictions thresholds
Detailed conviction information affecting the safety levels reported on the carrier profile.
Section 5: Inspections thresholds
Detailed inspection information affecting the safety levels reported on the carrier profile.
You can access your Carrier Profile report for free on the Internet by requesting an ID and password from Carrier Safety Programs.
