If you change your name and/or address, you must advise SGI of the changes in writing within 15 days, or call 1-800-667-4686 to register the change. Any changes to name and/or date of birth must be accompanied by a legal document supporting the change. This service can also notify several other provincial and municipal government organizations of your address change at the same time.
