To get a commercial licence - classes 1, 2, 3 or 4 or endorsements 1, 2, 3, 4, G or S - you need to submit a Commercial Driver's Medical Report form (pdf, 160 kb) before booking your written and road tests. Drivers will be notified once their medical has been reviewed.
Our Medical Review Unit (MRU) will review your medical report to ensure you meet the requirements for a commercial licence.
Maintaining your licence
Once you have a commercial licence, you must provide periodic medical reports to maintain your commercial driving privileges. This is a requirement under the National Safety Code Program and applies across Canada and the United States. Fulfilling this requirement allows Canadian drivers to operate vehicles across Canada and in the United States.
If you have 1 of the following medical conditions you are prohibited from operating in the United States and a W restriction will be added to your driver's licence:
- Drivers with insulin dependent diabetes.
- Drivers with an established medical history or clinical diagnosis of epilepsy.
- Drivers who do not meet the minimum hearing requirements to transport dangerous goods.
- Drivers currently operating under a medical waiver.
Note: If using the vehicle for personal use, the W restriction does not apply.
Medical reports are requested every:
- 5 years for drivers 18 to 45 years of age
- 3 years for drivers 46 to 65 years of age
- 1 year for drivers 66 years of age or older
How it works:
- When your medical report is due, we'll send you a medical report form.
- Return the form - completed and signed by your physician, nurse practitioner or occupational health nurse to us within 90 days.
We do not pay for any portion of a commercial driver's medical report. All medical information is treated as confidential as per The Health Information Protection Act.
Report results
Your ability to maintain a commercial licence depends on your medical report:
- If we receive your report on time and you meet the requirements for a commercial licence, your commercial driver's licence will not be affected.
- If you don't meet the requirements for a commercial licence, additional medical information may be required and your driver's licence could be restricted, suspended or changed to a Class 5 licence.
If you don't submit your report on time, your driver's licence will be downgraded to a Class 5 licence. If you're unable to meet the deadline, contact the MRU as soon as possible to request a deadline extension.
